Aim: to demonstrate the complexity of diagnosing Auditory Neuropathy Spectrum Disorder and to emphasize the importance of comprehensive audiological assessment. A case of late diagnosis of grade III bilateral sensorineural hearing loss in an 11-year-old child with preserved OAE and type A tympanogram is presented. Magnetic resonance imaging with contrast showed no pathology. Binaural hearing aid fitting was performed using Starkey Evolv AI 1600 devices with objective verification (REM). Results. Personalized fitting significantly improved speech intelligibility and social adaptation. Conclusion. Preserved OAE and normal tympanometry do not exclude severe hearing impairment at the neural level.
