The international cluster-randomized AFFIRMO trial demonstrated a neutral effect of an integrated digital care approach combining the integrated ABC pathway (iABC) and comprehensive geriatric assessment (CGA) on reducing unplanned hospitalizations among older patients with atrial fibrillation and multimorbidity. The absence of a significant clinical benefit may be attributable to the already high baseline uptake of oral anticoagulation across participating centers (97.3%) and the limited long-term engagement of older adults with the mobile application.
