Comparative Characteristics of Pathogens of Postoperative Infectious Complications in Patients with Combat-Related Injuries and Wounds in Ukraine

The problem of prevention and treatment of postoperative infectious complications in patients with combat-related Injuries and wounds remains relevant. Despite the use of modern methods of diagnosis, antibiotic prophylaxis, and treatment, the frequency of postoperative infectious complications does not tend to decrease. The aim: to analyze of microorganisms isolated from military personnel with postoperative infectious complications and to identify the main responsible pathogens of various type of healthcare associated infections (HAIs).The research methods. We retrospectively analyzed of pathogens isolated from military personnel with HAIs in 11 civilian hospitals in 6 regions of Ukraine during from January 1 to December 31, 2025. Identification of the species of isolated strains was carried out using automated microbiological testing (Vitek-2; bioMe’rieux, France). Results. A total 5,238 strains of microorganisms isolated from of 1,744 patients with HAIs were analyzed. The Gram-positive microorganisms composed 28.4% from all isolates and Gram-negative — 69.8%. In common etiology structure of HAIs predominated Escherichia coli (15.8%), Enterococcus faecalis (11.2%), Staphylococcus aureus (8.1%), Klebsiella pneumoniae (7.6%), Serratia marcescens (7.4%), Acinetobacter baumannii (6.3%), Pseudomonas aeruginosa (6.1%) and Proteus mirabilis (5.2%). Of all cases of HAIs, 82.6% were found to be polymicrobial infections. Conclusion. This study showed that in most cases, HAIs in patients with combat-related Injuries and wounds is polyetiological. The main pathogens of HAIs are E. coli, E. faecalis, S. aureus, K. pneumoniae, S. marcescens, A. baumannii, P. aeruginosa and P. mirabilis. The quantitative and qualitative characteristics of the isolates have significant differences depending on the specific type of HAIs. This justifies the need for systematic microbiological monitoring of microorganisms that contaminate the surgical site.
