Etiology of Healthcare-associated Infections in Patients with Combat-Related Injuries and Wounds

Infectious complications of surgical treatment of patients with combat-related injuries and wounds reduce the effectiveness of treatment and increase the duration of hospitalization and the cost of treatment. Despite the use of modern methods of diagnosis, antibiotic prophylaxis, and treatment, the frequency of postoperative infectious complications does not tend to decrease. The aim: to analyze the etiological structure of healthcare associated infections (HAIs) in patients with combat-related injuries and wounds, and to identify the main responsible pathogens. The research methods. We retrospectively analyzed pathogens isolated from patients with HAIs in 11 civilian hospitals in 6 regions of Ukraine during 2022–2025. Identification of the species of isolated strains was carried out using automated microbiological testing (Vitek-2; bioMe’rieux, France). Results. A total 17,286 strains of microorganisms isolated from 5022 patients with HAIs were analyzed. The Gram-positive microorganisms composed 23.4% from all isolates and Gram-negative — 72.9%. In common etiology structure of HAIs predominated Escherichia coli (17.1%), Pseudomonas aeruginosa (11.5%), Enterococcus faecalis (6.9%), Acinetobacter baumannii (6.9%), Stenotrophomonas maltophilia (6.8%), Klebsiella pneumonia (6.3%), Staphylococcus aureus (5.7%), Serratia marcescens (5.5%) та Enterococcus faecium (4.8%). Of all cases of HAIs, 87.9% were found to be polymicrobial infections. Conclusion. This study showed that in most cases, HAIs in patients with combat-related injuries and wounds is polyetiological. The main pathogens of HAIs are E. coli, P. aeruginosa, E. faecalis, A. baumannii, S. maltophilia, K. pneumonia, S.aureus, S. marcescens and E. faecium. The quantitative and qualitative characteristics of the isolates have significant differences depending on the geographical location of the hospitals where the patients underwent surgical treatment. This justifies the need for systematic microbiological monitoring of microorganisms that contaminate the surgical wound.
