The article presents a comprehensive pathogenetic analysis and systematization of modern scientific data on the use of systemic enzyme therapy, in particular the highly selective proteolytic enzyme serratiopeptidase (using the example of the drug Movinaza), in modern obstetric and gynecological practice. The molecular mechanisms of action of the enzyme, which include fibrinolytic, anti-edematous, anti-inflammatory and COX-independent analgesic vectors, are substantiated. Special attention is paid to a differentiated approach and phase-dependent safety monitoring in the management of patients with benign functional ovarian cysts, tubo-ovarian formations of inflammatory genesis. Integration of the drug Movinaza into complex treatment regimens and ERAS protocols allows reducing the risk of postoperative adhesiogenesis, preventing the chronicity of pelvic pain and increasing the effectiveness of concomitant antibacterial therapy due to the destruction of bacterial biofilms.
