The article reviews modern ideas about the pathogenesis of acute and chronic rhinosinusitis (RS) as a complex inflammatory process with activation of innate immunity mechanisms, cytokine cascade and interleukin (IL)-8-associated neutrophilic inflammation. It is shown that traditional therapy does not always provide complete control of persistent inflammation, especially in neutrophilic endotypes of chronic RS. Data on non-individualized homeopathic therapy and the efficacy of the complex drug Cinnabsin are presented. Experimental studies have proven the ability of the drug to reduce the synthesis of pro-inflammatory IL-8 and IL-1β, increase the level of anti-inflammatory IL-10 and γ-interferon, modulate the mechanisms of local immune defense. Clinical studies have demonstrated a decrease in the severity of the main symptoms of RS, an improvement in the rhinoscopic picture and a decrease in the frequency of relapses. The data obtained allow us to consider Cinnabsin as a drug with anti-inflammatory, secretolytic, secretomotor and immunomodulatory effects, which should be included in the complex therapy of RS.
