Vitamin D homeostasis in patients with chronic pancreatitis and exocrine pancreatic insufficiency with comorbidity with chronic obstructive pulmonary disease

Aim: to determine the features of vitamin D homeostasis with comorbidity of chronic pancreatitis (CP) and chronic obstructive pulmonary disease (COPD) in comparison with the isolated course of these diseases. Object and methods of the study. 145 patients were examined, including 35 patients with COPD, 35 patients with CP and 75 patients with CP with comorbid COPD. Results. In patients with COPD, the content of vitamin D in the blood was 24.8% lower than in practically healthy individuals, in patients with CP — by 54.4%, in patients with comorbidity of CP with COPD — by 75.2%. The maximum decrease in calcium content — by 14.3% — was observed in patients with CP with COPD. In patients with comorbid CP with COPD, compared with patients with an isolated course of the disease, the highest intensity of oxidative stress was found (an increase in the content of malonaldehyde in erythrocytes by 2.0 times and the level of diene conjugates by 1.9 times), as well as nitrosative stress (by the content of nitrites/nitrates in the blood) — by 2.6 times. The content of C-reactive protein in the blood was increased in patients with CP — by 13.3 times, in COPD — by 17.2 times, in CP with COPD — by 21.5 times, which indicates a higher degree of inflammation in patients with comorbidity. The content of fecal elastase-1 in patients with CP and with comorbid CP and COPD was reduced by 1.3 and 2.0 times, respectively, which indicates exocrine pancreatic insufficiency, which increased in the examined patients. Conclusions. Vitamin D deficiency in the blood significantly increases in comorbidity of CP with COPD, contributes to impaired calcium metabolism and correlates with an increase in the intensity of systemic inflammation, oxidative and nitrosative stress and the degree of exocrine pancreatic insufficiency.
