This article examines three challenging clinical cases presented at the ESC Congress 2026, illustrating the successful management of atrial fibrillation and atrial flutter in atypical clinical scenarios: thyroid storm, a perigraft hematoma, and a giant left atrial appendage aneurysm. The authors argue that addressing areas of uncertainty in international clinical practice guidelines requires flexibility in applying the CARE pathway, the systematic use of multimodality cardiac imaging, and decision-making by a multidisciplinary team.
