This review article examines the new clinical guidelines of the European Society of Cardiology (2026 ESC Guidelines on Cardiac Rehabilitation), which, for the first time in the Society’s history, have been issued as a standalone comprehensive document. The article systematizes updated indications and levels of evidence for a broad spectrum of conditions, including heart failure, atrial fibrillation, and cardio-oncology, and provides a detailed analysis of tailored approaches to prescribing aerobic and resistance exercise based on individual ventilatory thresholds. Particular attention is paid to the implementation of patient-reported outcome measures (PROMs) and patient-reported experience measures (PREMs), shared decision-making, and strategies for overcoming socioeconomic and gender-related barriers, illustrated by a clinical case study. The article also explores the integration of digital technologies into cardiac rehabilitation, including telemedicine, mobile applications, and artificial intelligence, highlighting their potential to guide the modernization of contemporary cardiovascular care.
